REGEN-D 60Basics of Burns
Burns are one of the most painful injuries to endure and more difficult for health caregivers to deal with.

Burns can be classified into the following types based on cause:
  • Scalds : Includes immersion in or splashing by hot liquids, grease, and steam
  • Contact burns : Includes touching a hot object or substance
  • Fires
  • Chemical burns
  • Electrical burns
  • Radiation
Burns are classified in to 3 types based on extent of injury
  • First Degree: Epidermis of the skin is involved, causing pain and redness
  • Second Degree: Dermis is involved, extends to the layer below the epidermis (the dermis), causing pain, redness and blisters that may ooze. Blisters are hallmark of 2nd degree burns.
  • Third Degree: Subcutaneaous tissues and deep tissues i.e. muscles and bones are involved. The burn site appears pale, charred or leathery and there is generally no sensation in the area because the nerve endings are destroyed.
India has a high incidence of burn injuries with around 7-8 lacks burn admissions annually. Hence burn injuries have become a major public health concern to handle.

Do's and Don'ts to guide you towards safety
Do's
  • First stop the burning process / hot surface contact
  • Reassure and calm down the victim
  • Immerse or run water on the burnt surface till the victim calms down / is comfortable
  • Always clean the burnt area with soap solution, to get rid of microbes / contaminants
  • Always cover the burnt area with sterile / clean and moist cloth
  • Invariably elevate the affected part above the level of heart
  • Severely burnt victims must be laid on a clean surface and give leg elevation
  • Always lay the victim sideways (laterally) in cases of inhalational burns. E.g. gasping patient
  • Seek professional medical advice / care immediately
Don'ts
  • Do not cover the victim with a blanket / rug / thick cloth, except to put off the flames
  • Do not remove the clothes / dressings that are adherent to burnt surface
  • Do not use ice or ice cool water to cool down the burnt area. The ideal temperature of water should be 15°C
  • Do not use home remedies / unregistered products for local application on burnt area
  • Never feed orally a severely burnt victim i.e. > 30%, in spite of severe thirst
Special concerns
Burns on face and neck: The victim must be rushed to a burn center. Neglecting it results in contractures, disfigurement and scarring.

Burns across a joint: After the first aid they should seek medical advice / care as they heal with contractures, which result in decreased range of motion across the joint.

Burns over genetilia: Medical care should be taken even if the burn is small as it can get infected very quickly.

Call immediately for Emergency medical assistance, if
  • The burn is extensive or severe
  • There is a chemical or electrical burn or uncertainity about the burns severity
  • The victim shows signs of shock
  • The victim has an airway burn
